she is about 4.5 5 months old
she and several others from the same home lived with a man who was loving but could not take care of the dogs or himself (one of our volunteers paid for all their vetting and paid for him to enter a alcohol rehab center)

breed is a guess
she will be a medium size dog though --- vet thought this based on her current size
